DESCRIPTION OF WORK This position focuses on assessing local health needs, planning community programs, and delivering direct public health services within Berks County. As a Community Health Nurse, you will perform the following duties: Community Assessment: Review county health data to identify needs and guide public health activities Program Development: Create initiatives based on protocols, guidelines, and community priorities Health Monitoring: Track communicable diseases and enter data into state health systems Public Education: Provide information on health topics to support community awareness Outreach Services: Offer case management and support to individuals and targeted groups Clinical Care: Deliver immunizations and conduct disease-related specimen collection according to guidelines Interested in learning more? Work Schedule and Additional Information: Full-time employment, 37.5 hours per week Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch. Work infrequently requires statewide travel and irregular or flexible work hours, including overnights, weekends, and holidays, in accordance with policy. REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY QUALIFICATIONS Special Requirements: This position requires active authorization to practice as a Registered Nurse in Pennsylvania.
